1. Who we are
Mortal Healthcare is a non-clinical telehealth access program operated by Mortal Science LLC, a Texas limited liability company. This policy describes how we collect, use, and protect information when you use mortalhealthcare.com and the non-clinical services we provide.
2. Information we collect
- Account information: name, email address, phone number, shipping address, and date of birth.
- Health information: the information you provide in your health intake and through the patient portal, which is shared with the independent licensed provider who evaluates you.
- Payment information: processed by our payment processor. We do not store full card numbers.
- Usage information: standard log data such as browser type, pages visited, and IP address.
3. Protected health information and HIPAA
Health information you provide in connection with care is protected health information under the Health Insurance Portability and Accountability Act (HIPAA). Your care is provided by independent licensed healthcare providers, who are responsible for their own HIPAA notices of privacy practices. Mortal Science LLC handles protected health information as a business associate of those providers and operates under business associate agreements with the parties in the care chain. We use and disclose protected health information only as permitted by HIPAA and those agreements: for treatment, payment, and healthcare operations, and as required by law.
4. How we use information
- To enroll you and route your intake to an independent licensed provider
- To coordinate pharmacy fulfillment and shipping if a prescription is issued
- To process payments, provide customer support, and maintain your account
- To operate, secure, and improve the website and services
- To comply with legal obligations
5. How we share information
We share information with the independent licensed providers who evaluate and treat you, with the licensed pharmacies that fill prescriptions, with payment processors, and with service providers who support our operations under confidentiality obligations. We do not sell personal information, and we do not share health information with employers or referral partners. We may disclose information when required by law.
6. Security
All transactions and all pages handling customer, patient, or medical information are encrypted in transit using SSL/TLS. Access to health information is limited to personnel and service providers who need it to perform their roles.
7. Retention
We retain information for as long as needed to provide services and to meet legal, regulatory, and record-keeping requirements, then delete or de-identify it.
8. Your choices and rights
You may access and update your account information through the patient portal, and you may contact us to request access to, correction of, or deletion of your personal information, subject to legal retention requirements. Rights in your medical records under HIPAA are exercised through your treating provider. Depending on your state, you may have additional privacy rights; we honor verified requests as required by applicable law.
9. Children
The services are for adults 18 and older. We do not knowingly collect information from anyone under 18.
10. Changes
We may update this policy from time to time. The date above reflects the current version. Material changes will be posted on this page.
