Compounded Medication Disclosure

Compounded medications are custom-prepared medications prescribed for an individual patient when a licensed provider determines they are clinically appropriate.

Compounded medications:

• Are not FDA-approved
• Are not evaluated by the FDA for safety or effectiveness
• May differ from FDA-approved drugs in formulation, strength, stability, or availability
• Are prepared by state-licensed pharmacies or FDA-registered outsourcing facilities

Compounded medications are prescribed solely at the discretion of an independent licensed provider.
